Eureka Public Schools

To complete an out of district registration, please click on the link to open the registration form.

Instructions:

1. Download and print the application.

2. You only need to fill out the 1st Section at the top of the page (highlighted in yellow)

3. Return back to the school in one of these options:
Mail to Eureka Public Schools, PO Box 2000, Eureka MT 59917
Email to the District Secretary at oescobar@teameureka.net
Drop off to a school building principal for grade level of student (High School, Middle School, Elementary)

4. Once the school receives it, You will be mailed back a letter indicating the application has been obtained.
